Bioidentical HRT in Sun City: A Patient's Guide to Hormone Replacement Therapy

A plainspoken guide to hormone replacement therapy from Dr. Annie Khurana — who it helps, what symptoms it addresses, when it's appropriate, and what to expect at your first visit.

Over the last few years, more patients in Sun City have walked into my office and said some version of the same thing: "I don't feel like myself anymore." They are sleeping poorly, struggling with mood swings, feeling foggy, or simply running out of energy by mid-afternoon. Often, the underlying story is hormonal — and often, hormone replacement therapy (HRT) can help.

This guide is meant to give you a clear, honest picture of what HRT is, who it tends to help, and what a thoughtful evaluation looks like. It is not a sales pitch. Hormone therapy is a medical decision, and it deserves a medical conversation.

What HRT actually is

Hormone replacement therapy replaces hormones your body is no longer making in the amounts it once did. For women, that usually means estrogen, progesterone, and sometimes a small amount of testosterone. For men, it usually means testosterone. "Bioidentical" simply means the hormone molecule is chemically identical to what your body makes on its own. Many FDA-approved hormones are bioidentical; compounding pharmacies can also prepare bioidentical formulations when a standard product is not the right fit.

Who tends to benefit

HRT is most commonly used in two groups:

  • Women in perimenopause or menopause who are dealing with disruptive symptoms — typically anywhere from the early 40s into the 60s.
  • Men with low testosterone confirmed by blood work, especially when symptoms are clearly affecting quality of life.

I also see patients who have had a surgical menopause, certain cancer treatments, or early hormone decline. Each situation deserves its own evaluation.

Symptoms HRT can address

The symptoms I hear about most often include:

  • Hot flashes and night sweats
  • Sleep that is light, broken, or simply not restorative
  • Mood changes — irritability, anxiety, low mood
  • Brain fog and difficulty concentrating
  • Low libido and changes in sexual function
  • Fatigue that does not improve with rest
  • Joint aches and changes in body composition

Not every symptom is hormonal, which is exactly why a careful evaluation matters. Thyroid issues, sleep apnea, depression, and medication side effects can all look similar.

When HRT is appropriate — and when it isn't

HRT can be a strong option when symptoms are meaningfully affecting daily life and when your medical history supports it. For many women, starting HRT closer to the onset of menopause appears safer than starting many years later. For men, treatment is appropriate when blood work confirms low testosterone on more than one occasion and symptoms line up.

HRT is generally not the right choice when there is a history of certain hormone-sensitive cancers, active blood clots, untreated heart disease, or unexplained vaginal bleeding. Men with certain prostate conditions or untreated sleep apnea also need careful evaluation before starting therapy. None of this is meant to scare you — it is meant to make the point that hormones are powerful, and the right answer depends on your full picture.

What your first visit looks like

At Pinnacle Health, a first HRT visit is unhurried. We talk through your symptoms, your medical and family history, and the goals that brought you in. From there I usually order targeted lab work — hormone levels, thyroid, metabolic panel, and other markers based on your story. We review those results together at a follow-up visit and build a plan if HRT makes sense. If it doesn't, we talk through other options.

If we move forward, treatment is monitored closely. Doses are adjusted based on how you feel and what your labs show. Hormone therapy is not "start it and forget it" — it is an ongoing conversation.

What to take away

If you have been quietly putting up with symptoms because you were told they were "just aging," please know there is often more we can do. A good HRT evaluation is personal, thorough, and grounded in your history — not driven by a trend or a checklist.

You can read more about our approach on the Hormone Replacement Therapy page, or call the clinic to set up a consultation.
